The Individualized Education Plan (IEP) is the basis of all Special Education services. It is at the IEP meeting that the IEP Team (IEPT) meets to discuss the student's strengths, needs, and ultimately what services will be needed so the student can progress in the school curriculum. The LISD uses IEP forms that were created by the State of Michigan. To download the following forms, right click on the title and select "Save Target As...". Once saved on your computer, you will be able to open and edit the input fields with Adobe Reader 8.0 or greater.
