PLEASE NOTE:

Scholarship applications may be downloaded, printed, then submitted with other documentation that is asked for in the application.

Applications MUST be computer generated or typewritten. They will be disqualified if handwritten.

Cover sheet should be removed when submitting your application.

Multiple applications can be submitted together in one envelope (please don't fold applications)

Paper clips are preferred over staples. Please - NO notebooks, presentation sheets, or sheet protectors
